Request & Complete the Revocable Living Trust Intake Form

Request an intake form by submitting the form below, or call the office. We can email you a link to complete the intake form online, or we can email or mail a PDF version of the intake form.

In order to schedule an appointment with the Attorney, you will need to complete certain sections of the intake form.  An appointment will not be scheduled until you submit the intake form with the required sections completed.

Request an Intake Form

If you do not receive the intake form within 24 business hours, please confirm that the email is not in your spam folder or call the office for assistance.

Appointment With Attorney

Once you have completed the required sections of the intake form, you can schedule an appointment with the Attorney.  The appointment can take place in-person, zoom, or over the telephone.

During this meeting, the Attorney will go over the intake form with you, any questions that you may have, and discuss various estate planning options to meet your needs.

The fee for this meeting is $350 and this is credited towards the cost of your Estate Plan, so long as you sign the retainer agreement within one (1) month of meeting with the Attorney.

The Lepe Law Firm only extends one (1) initial consultation per legal issue. In order to speak with the Attorney after your initial consultation, you will need to retain the Attorney. Therefore, we recommend that you do not schedule your consultation until you are ready to proceed with your Estate Plan.

Sign Retainer Agreement & Pay

After the meeting with the Attorney, you can decide to retain our office at that time or contact our office at a later time.

You will only receive credit for the consultation if you retain our firm within one (1) month of the initial consultation.  As a reminder, the Attorney does not extend follow-up consultations.  In order to speak with the Attorney after the initial consultation, you will need to retain our office.

Half of the total cost of your Estate Plan is required at the time of the signing of the Retainer Agreement – pricing is listed below.

Your Trust Signing Appointment is scheduled once you sign the Retainer Agreement and make payment. We normally book Trust Signing Appointments 3-6 weeks from when the retainer agreement is signed.

Trust Signing Meeting

The Trust Signing Meeting takes place at our office in Claremont, CA. The Attorney’s schedule does not permit her to travel to you. Virtual trust signing appointments are available, but additional fees will apply.

Single Person:      1 – 1.5 hours
Married Couple:    2 – 2.5 hours
